As a reminder, districts that use their EETT Formula Technology Plan for their E-rate plan must also fill out the E-rate Supplemental Budget Analysis Sample form each year that they are applying for E-rate.

This form is available from the CDE website. This form should be completed annually and kept on file in the district.
